How it Works
Invite
Embed a link in a pre-visit email or text message. Or provide a QR code in your office when patients arrive for easy access to the risk assessment questionnaire.
Assess
Patients can complete the questionnaire on their phone in five minutes or less. Answers are scored against national cancer care guidelines and the patient immediately learns if they meet testing criteria.
Summarize
You receive a summary of the assessment indicating if the patient meets criteria for genetic testing, listing individual responses and flagging which criteria were met, all of which can be used to support you in delivering follow-up care.
Triage
If a patient meets criteria for genetic testing, you can immediately triage them to schedule an appointment with a Genome Medical genetic counselor. Appointments are available in less than a week.
